Case Management coordinates medical and nursing care in close collaboration with the treatment team (doctors, nurses, therapists, social services), taking into account the individual needs of the patient. The case managers thus serve as patient navigators.

The Case Management staff serve as points of contact for

• patients
•  family members
• home care services
• primary care physicians and specialists, as well as
• care facilities

Responsibilities of Case Management

Case Management coordinates the admission date and ensures that everything is prepared for the stay. Accommodation preferences, such as a comfort room, are also taken into account. All appointments necessary for treatment at the hospital are coordinated. 

Discharge and the transition from the hospital to your own home or to a facility providing further care is coordinated by our staff as part of discharge management(subject to consent).
