Case management - your patient guide
Our case manager serves as the point of contact for you, your family members, home care providers, primary care physicians, specialists, and long-term care facilities. She coordinates a patient’s admission date and ensures that everything is prepared for their stay. This includes accommodating requests regarding room assignments, such as a comfort room. All appointments necessary as part of the hospital treatment are coordinated. She also handles discharge and the transition from the hospital back to the patient’s own home or to a facility providing further care.
Data protection
We take data protection very seriously. We treat your personal information and documents as strictly confidential. We process and store your data only to the extent necessary for the provision of your treatment. Your data is primarily processed and archived automatically. Data is disclosed to third parties exclusively on the basis of a legal provision (e.g., the data transfer to statutory health insurance as stipulated in Section 301 of the German Social Code, Book V [SGB V], for the billing of treatment) or after you have consented to the disclosure (e.g., a discharge letter to your primary care physician). Whenever possible, your data is processed anonymously (e.g., for internal statistical analyses). Once the storage of the data is no longer necessary, it is deleted. We have implemented technical and organizational security measures to protect your data from loss, destruction, manipulation, and unauthorized access. You have the right to request information about the data stored regarding you.
You can find comprehensive information in our

Electronic patient file (ePA)
Information regarding the electronic patient record (ePA)
(pursuant to Sections 346, 347, 348 of the German Social Code, Book V)
Dear Patients,
this information applies to you if you have an electronic patient record (ePA). The ePA is a record that is solely your responsibility. This means that you alone decide whether information is stored in your ePA, who may view and access it, and whether information is deleted.
Storage of particularly important information, unless you object
If you are already using an ePA, please let us know. We will store certain information about your current treatment that is particularly important in your ePA. You do not need to request this from us. We will do this automatically for you. This includes, among other things, the following information regarding your current stay or treatment with us:
- Data on laboratory results,
- Reports from diagnostic imaging,
- Reports from medical examinations and procedures, and
- discharge summaries/physician’s letters.
The same applies if we have information from previous treatments at our facility and it seems appropriate to us, in light of your ongoing medical care, to also store this in your ePA.
If you do not wish for this information to be transferred and stored in your ePA, we will of course not do so. Please let us know! Thank you very much!
Transfer and storage of additional information in the ePA at your request
If you would like additional information beyond the above-mentioned details to be stored in your ePA, please let us know! Provided we collect and electronically process this information as part of your current treatment, we will also store it in your ePA.
Electronic emergency data
Electronic emergency data can also be stored in your ePA as soon as this is technically possible. If we change any information in your emergency data as part of your current treatment, we will also update the emergency data stored in your ePA.
Right to object, erasure, or restriction
You have the right to object to the storage of information in your ePA.
In addition, you can delete any information stored in your ePA or entered by us yourself.
Instead of deletion, you can also restrict editing or readability. A “restriction” offers the advantage that the information is stored in your ePA and can be used by you, but cannot be viewed by everyone or only by you. You alone decide whether only you can see all the information or who can view which information. You can set this yourself via your smartphone, PC, laptop, or similar device.
Documents Made Visible Later
If information in your ePA is not visible to us at the time of admission to our facility because our access is restricted, please note the following: If information that was not visible to us at the time of admission is made visible to us at a later date, we will not receive any notification of this.
Please feel free to contact us if you have any further questions.
Tariff of charges
You will find an information sheet about fees in your patient information folder.
Discharge management
Discharge management provides important support for follow-up care after a hospital stay, especially for older and single individuals. For many patients, treatment does not end on the day of discharge; they continue to require what is known as follow-up care.
The organization of follow-up care through discharge management includes, among other things:
• Medications and a certificate of incapacity for work
• Assistive devices, e.g., walking aids
• Medical care provided by a general practitioner or specialist in
private practice • Home care provided by a nursing service
• Inpatient care in a nursing facility
• Rehabilitation stays
By providing early assistance and measures, the goal is to ensure as seamless a transition as possible to follow-up care.
Dismissal
Your discharge has been medically authorized. You will receive your discharge summary and a preliminary medical report on your ward. If you have not yet paid your copayment, you can do so at the Patient Administration office.
Food
The food service assistants ask patients about their meal preferences on weekdays, the day before. Each day, you can choose from three different lunch options: regular meals, light meals, and vegetarian meals. Unless patients have dietary restrictions due to specific diets or medical tests, they are free to choose from the available options. They may also substitute individual components within the menu options.
In the event of dietary restrictions, the basis of the lunch menu is always Menu 2 (“light full-course meal”). In this case, your choice of components is unfortunately somewhat limited. For breakfast and dinner, the GPR Clinic offers patients either various options to choose from, or you can order à la carte and put together your own breakfast and dinner. The selection has been expanded to include a Mediterranean option as well as Turkish baked goods.

Questionnaire
Shortly before your discharge, the nursing assistant on the ward will give you a patient questionnaire. You can drop your completed questionnaire directly into the suggestion and complaint box on the ward. We are pleased if you were satisfied with our care, but we also appreciate constructive criticism. We would appreciate it if you would answer our questions! If you provide your contact information on the questionnaire, we will be happy to reach out to you for further clarification.
The patient’s perspective provides valuable insights into the quality of care at the hospital and can highlight areas for improvement. The CLINOTEL hospital network, of which the GPR Clinic is a member, therefore tracks patient satisfaction at its member hospitals through an ongoing patient survey and publishes the results on its website.

If there are no taxis at the taxi stand across from the GPR Clinic, please ask the staff at the information desk; they will be happy to call one for you. However, if you need medical transportation, your care team will be happy to assist you.
Telephone
In September 2023, a new comprehensive patient communication system was launched, enabling the GPR to transition to a new fee model. Please note that these services (TV, radio, telephone, and Wi-Fi) are subject to a fee and can now only be booked as a package. The fee is €4 per day. Booking is done at patient registration by signing a consent form.
