The First Department of Internal Medicine provides general internal medical care in Rüsselsheim and the surrounding area, particularly for acutely ill patients, and has 84 beds. In addition to inpatient care, the GPR Clinic places a special emphasis on outpatient services.

The Internal Medicine Emergency Department, the Endoscopy/Ultrasound Unit, the Gastroenterology/Hepatology Functional Diagnostics Unit, and the Gastroenterology/Hepatology, Oncology, and Rheumatology Outpatient Clinics are integrated into the hospital.

Gastroenterology

In the field of gastroenterology, we treat conditions such as chronic inflammatory bowel diseases and infectious diseases using modern diagnostic procedures to assess the function of the gastrointestinal tract, such as breath tests. Comprehensive endoscopic examinations (including gastroscopy and colonoscopy) are performed, including the use of new methods (e.g., endoscopic removal of gallstones from the bile duct, placement of special tubes in the bile duct or gastrointestinal tract to bypass tumor sites). Modern ultrasound equipment and the use of contrast-enhanced sonography enable the detection of even small pathological changes and often make it clear during the examination whether the changes are benign or malignant.

Hepatology

Hepatologyfocuses on the diagnosis of acute and chronic liver diseases. The liver biopsy, which is sometimes necessary for this purpose, can be performed on an outpatient basis if needed. With the use of ultrasound, even the smallest lesions in the liver—as well as in other organs—can be biopsied.

Cancer diseases

A particular focus of the First Department of Internal Medicine is the treatment of patients with cancer using chemotherapy and/or immunotherapy (medical oncology). The department also offers services for the diagnosis of blood disorders through microscopic examinations and specialized methods (hematology). For several years now, local treatment of liver tumors using heat (radiofrequency ablation) has also been possible. In cooperation with the Institute of Radiology and Nuclear Medicine, local treatments involving the injection of anticancer drugs into the hepatic artery (chemoembolization) are offered.

Furthermore, the First Department of Internal Medicine places a special emphasis on the prevention of colorectal cancer, the second most common type of cancer in industrialized countries. A prerequisite for the success sought with this measure is increased participation by the population in screening examinations.

Special isolation rooms are available for patients with infectious diseases (e.g., tuberculosis).

Diabetes and nutrition counseling provides diagnostic and therapeutic care for patients with diabetes (diabetology).

With the establishment of the Department of Internal Medicine and Rheumatology, the diagnosis and treatment of inflammatory rheumatic diseases (mostly so-called autoimmune diseases) are offered on an outpatient and inpatient basis.

Gastroenterology / Hepatology outpatient clinic

Gastroenterology focuses on the diagnosis and treatment of diseases of the abdominal organs, such as the stomach, intestines, liver, gallbladder, and pancreas.

Diseases of the abdominal organs, e.g.:
• Colitis and Crohn’s disease (consultation with immunosuppressive therapy (infliximab, adalimumab, vedolizumab))
• Hepatitis C (consultation with DAA-based therapy, interferon-free)
• Hepatitis B
• Autoimmune hepatitis
• Metabolic liver disease

Diagnostics are performed using:
• Laboratory tests
• Breath tests (C13, lactose, fructose, glucose)
• Ultrasound, color Doppler ultrasound
• Contrast-enhanced ultrasound
• Liver biopsy and ultrasound-guided punctures
• Complex therapeutic gastroscopy with dilation of strictures and variceal ligation, removal of polyps
• Colonoscopy
• Consultation regarding indications for endosonography and ERCP
• Consultation regarding indications for therapeutic procedures (endoscopic full-thickness resection, mucosectomy, etc.)
• Palliative drainage of ascites and pleural effusions using a tunneled catheter (Pleurx)

The focus of our oncology department is on the diagnosis, treatment, and follow-up care of tumors and diseases of the hematopoietic system. The range of treatments includes tumors of all organ systems, benign and malignant blood and lymph node disorders, as well as bleeding disorders.

Information:

Immunotherapies hold the potential to cure cancer. Pathologically complete remissions of over 70%, with long-term remissions amounting to a cure, are no longer rare. Unfortunately, immunotherapies are often discontinued due to side effects. The administration of glucocorticosteroids is detrimental to the immune response. Interestingly, it is precisely those patients who experience side effects who respond best to treatment. For this reason, the event will focus on the management of immune-related adverse events (irAEs). Particular attention will be given to the immunological fundamentals (avoidance of antibiotics, vitamin D supplementation, biologics instead of glucocorticosteroids). The focus will be particularly on endocrine, pulmonary, hepatic, enteric, and dermatological side effects. Bispecific antibodies and their combinations will also be discussed. Another key program item addresses an innovative care concept that envisions infrastructural adjustments in the emergency department (triage, regular continuing education on irAEs, optimization of oncological expertise) alongside closer integration of community-based colleagues and support from Mainz University Medical Center.
