Your most frequently asked questions about discharge management

Discharge management provides vital support for follow-up care after a hospital stay, especially for older and single individuals. For many patients, treatment does not end on the day of discharge; they continue to require what is known as follow-up care.

The organization of follow-up care through discharge management includes, among other things:

  • Medications and a certificate of incapacity for work
  • Medical aids, e.g., walking aids
  • Medical care provided by a general practitioner or specialist in private practice
  • Home care provided by a nursing service
  • Inpatient care in a nursing facility
  • Rehabilitation stays

The provision of early assistance and measures is intended to ensure as seamless a transition as possible to follow-up care.

Patients

  • who are insured by a public health insurance plan 
  • AND require follow-up care

are entitled to discharge management services during inpatient or day-care treatment at GPR Klinikum. 

Patients (or a legal representative) must give their written consent to participate in discharge management in advance (for data protection reasons). To this end, every patient with statutory health insurance receives a form upon admission to the GPR Clinic.

  • Participation in discharge management is voluntary.
  • Consent may be given retroactively and/or revoked at any time.

Assessment 

After a patient is admitted, GPR Klinikum assesses whether follow-up care is likely to be needed. We also monitor the patient’s health status to adjust care as necessary.

 

Planning

If the patient requires follow-up care, a discharge plan is created. The GPR Clinic’s discharge management team coordinates and organizes all necessary follow-up measures according to individual needs—in close consultation with all parties involved.

In doing so, the discharge management team consults internally with the individuals involved in the treatment process, such as:

  • Case managers 
  • Social services staff
  • Doctors
  • Therapists


For coordination purposes, the discharge management team contacts the following external parties as needed:

  • private practitioners
  • Follow-up facilities (rehabilitation, inpatient care, outpatient care)
  • Health insurance companies 
  • Family members or legal guardians



Discharge

Prior to discharge, the attending physician conducts a medical discharge consultation. On the day of discharge, the patient (and, with prior consent, the physician providing follow-up care) receives a preliminary discharge summary. This includes important information such as diagnosis, treatment, and medication. The contact information for the discharge management coordinator at GPR Klinikum is also noted on the discharge summary for any follow-up inquiries.

No, participation is voluntary.

If you do NOT wish to participate in discharge management, do NOT give your consent to GPR Klinikum.

Your options: 

  • You decide to give consent—but you can revoke it at ANY TIME! 
  • You do NOT give consent—but you have the option to provide consent for discharge management at any time.
     

If a patient requires further care, refusing consent may result in treatments NOT being initiated early enough or in a timely manner.

 

In order for GPR Klinikum to carry out discharge management, we require the patients’ written consent.

This is due to data protection regulations; since the GPR’s discharge management team may need to contact doctors, therapists, and health insurance providers, among others, to organize follow-up care. In doing so, patient data may need to be shared. This requires written consent.
